Completion of Part I of an Export Health Certificate (EHC) for the European Union (EU)

I.1 Consignor

Enter the name, address, country and telephone number of the Consignor (person who sends the consignment, or their representative) of the export consignment.

When entering the country, enter ‘United Kingdom’ if the Consignor is in England, Scotland, Wales, Northern Ireland or the Crown Dependencies of Jersey, Guernsey and the Isle of Man.

I.5 Consignee

Enter the name, address, country and telephone number of the Consignee (person to whom the consignment is shipped, or their representative) of the export consignment.

For consignments in transit through the European Union (EU), this information is not compulsory.

I.6 Person responsible for consignment in the EU

This field only needs to be completed for consignments transiting the EU which are destined for a non-EU country.

Enter the name, address, postal code and telephone number of the person responsible for the consignment when it is presented at the Border Inspection Post in the EU, and who will make the necessary declarations to the competent authorities on behalf of the importer.

I.8 Region of origin

This field can be left blank.

I.9 Country of destination

Indicate the name and the ISO code of the EU Member State of destination of the consignment:

For consignments in transit, indicate the name of the non-EU country of destination.

I.10 Region of destination

This field can be left blank.

I.11 Place of origin

Enter the name, address and the approval number of the place of origin of the consignment.

I.12 Place of destination

This field only needs to be completed for consignments transiting the EU which are destined for a non-EU country.

Enter the name, address, and postal code of the warehouse in the EU. Also, enter the approval number assigned by the EU to this warehouse. The individual responsible, listed at I.6, should be able to obtain this number for the exporter.

I.13 Place of loading

Enter the address and approval number of the place of loading of the consignment.

I.14 Date of departure

Enter the date of departure from the port of embarkation or from the place of loading in the United Kingdom. Enter the date in the format: dd/mm/yyyy.

I.15 Means of transport

Indicate only one means of transport from the UK to the EU and its identification (if by air, the flight number; if by water, the name of the ship).

Documentary references.

Optional: Indicate the number of the airline bill or the bill of lading.

Box I.16 Entry BIP in EU

Indicate the name and number of the Border Inspection Post (BIP) as it appears in the Official Journal of the European Union: https://ec.europa.eu/food/animals/vet-border-control/bip_en

Box I.17 No(s) of CITES

The CITES permit number is relevant only for animals and products listed in the Washington Convention on protected species. Except for game trophies, leave it blank.

Box I.18 Description of commodity

Provide a general description of the goods, which must correspond to the HS code to be entered at I.19.

Box I.19 Commodity code

If the Harmonized System (HS) code as it appears in the Official Journal of the European Union is not pre-printed on the certificate, indicate the appropriate code from the notes of the certificate. Box I.20 Quantity

Enter the total gross and net weights, if different, in kilograms (kg).

Box I.21 Temperature of product

Check the box for the appropriate temperature for transport/storage of the product.

Box I.22 Number of packages

Enter the total number of packages of products.

Box I.23 Identification of container/Seal number

Enter all the identification numbers of the seals and containers (if there is no regulatory requirement, this information is optional). If not known at point of application this information can be added by the certifying officer at the time of certification.

This applies only to products. Enter a description of the type of packaging from the following options:

Please indicate the intended use of the products (from the options appearing in each specific certificate).

Box I.26 For transit to 3rd country via the EU

If the final destination is the EU, leave blank.

If the animal products originating from the UK are exported to a non-EU country, and only transiting through the EU, indicate the name and the ISO code of the country of destination.

Box I.27 For import or admission into EU

For import into the EU, check the box. For transit through the EU, leave it blank.

For Equine imports into the EU only, additionally check one of the following options:

  • Definitive import: Imports of:
    • registered horses, registered equidae and equidae for breeding and production (Section A of Part 3 of Annex II to Commission Implementing Regulation (EU) 2018/659)
    • equidae for slaughter, respectively with or without passing through a marshalling centre (Section B of Part 3 of Annex II to Commission Implementing Regulation (EU) 2018/659)
  • Horses Re-entry: re-entry of registered horses for racing, competition and cultural events after temporary export to approved non-EU countries for a period of not more than
    • 30 days, (Section A of Part 2 of Annex II to Commission Implementing Regulation (EU) 2018/659)
    • 90 days to participate in specific competitions (Chapter 1 of Section B of Part 2 of Annex II to Commission Implementing Regulation (EU) 2018/659)
    • 90 days to participate in specific races (Chapter 2 of Section B of Part 2 of Annex II to Commission Implementing Regulation (EU) 2018/659)
  • Temporary admission horses: temporary admission for less than 90 days of registered horses (Section A of Part 1 of Annex II to Commission Implementing Regulation (EU) 2018/659).
