Health and Safety Executive User Research Participant Consent Form
Thank you for agreeing to take part in our user research. The Health and Safety Executive (HSE) does regular research to find out how people use our existing services and what they need from new services. This helps us create services that work better for the public and for government. In order to undertake this research and record the session (video and notes) we require your consent.
Please read these statements and sign at the bottom if you agree. If you have any questions, please ask.
Participation is voluntary I understand that taking part in the research is voluntary. I can withdraw my consent and stop at any time.
Collecting research data I understand that HSE may make sound and/or video recordings, take photos, make copies of documents, and take detailed notes of my research session. Staff from HSE and other organisations working with HSE will review this research data. HSE will store the research data securely for up to 2 years.
Data in prototypes I understand that if I enter any personal data into a prototype of a product or service, HSE will delete the data immediately after my research session.
Sharing research extracts I understand that HSE may extract sound clips, images and extended quotes from the research data for my session. HSE may show these extracts to staff from HSE and other organisations working with HSE so they can learn from what happens during my research session. HSE will store the extracts securely and may keep them indefinitely.
Storing research data I understand that HSE may store the research data and extracts outside the European Economic Area (EEA).
Legal disclosure I understand that HSE may release the research data from my session if there is evidence of wrongdoing or if required by law or by any competent authority.
Publishing findings I understand that HSE may make the findings of the research public so that anyone can use and learn from the results. The findings HSE makes public will not contain any of my personal data or anything that could identify me.
Confidential I understand that the ideas, designs and services I see in my research session are confidential. I will not tell anyone else about the ideas, designs or services until they are made public. I will not take away copies of any materials I use in the session.
